In the STEP 1 clinical trial, adults with obesity taking Injectable Wegovy® 2.4 mg achieved an average weight loss of 14.9% (approximately 33.7 pounds) over 68 weeks, compared to 2.4% on placebo.

In the SELECT trial, Injectable Wegovy® reduced the risk of major adverse cardiovascular events (heart attack, stroke, or cardiovascular death) by 20% in adults with established heart disease and obesity over a median of 39.8 months. The trial also showed a 15% reduction in cardiovascular death and a 19% reduction in death from any cause.

More than two-thirds of participants in STEP trials achieved at least 10% body weight reduction, and approximately one-third achieved at least 20% weight reduction with Injectable Wegovy® 2.4 mg.

Frequently asked questions about Injectable Wegovy®

How is Injectable Wegovy® different from Oral Wegovy® or other GLP-1 medications?

Injectable Wegovy® and Oral Wegovy® contain the same active ingredient (semaglutide) and are both FDA-approved for weight management and cardiovascular risk reduction. The key differences are in administration and convenience. Injectable Wegovy® is taken once weekly at any time of day, with or without food, and has the most extensive clinical evidence base including the SELECT cardiovascular outcomes trial.

Oral Wegovy® is a daily pill that requires taking on an empty stomach with no more than 4 oz of water, then waiting 30 minutes before eating, drinking, or taking other medications—which can be challenging for some patients. Injectable Wegovy® also has more predictable absorption and is the only formulation approved for adolescents. Compared to other GLP-1s, Injectable Wegovy® is the only weight loss medication proven to reduce cardiovascular events in patients with established heart disease.

How long does it take to work?

Most patients notice appetite changes within the first few weeks. Meaningful weight loss typically becomes visible by 8-12 weeks, with results continuing to build over time. The STEP trials showed peak weight loss at approximately 68 weeks of treatment.

Can I switch from Injectable to Oral Wegovy® later?

Yes, but it should be done under clinician guidance. Your General Medicine clinician can help you transition between formulations based on your goals, lifestyle, and how well you're tolerating treatment. Keep in mind that switching requires adjusting to a new dosing schedule and routine.

Is Injectable Wegovy® safe long-term?

Injectable Wegovy® has been studied in clinical trials up to 68 weeks with a well-characterized safety profile. The SELECT cardiovascular outcomes trial followed patients for a median of 39.8 months (over 3 years), demonstrating sustained safety and cardiovascular benefits. Long-term use should always be supervised by a clinician who can monitor your health and adjust treatment as needed.

Will insurance cover Injectable Wegovy®?

Insurance coverage varies by plan. Many commercial insurance plans cover Injectable Wegovy® for weight management in adults with obesity with at least one weight-related comorbidity. As of 2024, some plans also cover it for cardiovascular risk reduction in adults with established heart disease. Medicare coverage for weight loss is limited, though some Medicare Advantage plans may cover it for cardiovascular indications. What's the difference between Injectable Wegovy® and Ozempic®?

Both contain semaglutide, but they have different FDA-approved indications and dosing. Injectable Wegovy® (up to 2.4 mg weekly) is approved for chronic weight management and cardiovascular risk reduction in patients with obesity. Ozempic® (up to 2 mg weekly) is approved for type 2 diabetes, with indications for blood sugar control, cardiovascular risk reduction, and kidney disease protection in patients with diabetes. While both cause weight loss, Ozempic® is not FDA-approved for weight loss in patients without diabetes.

What are the side effects?

The most common side effects of Injectable Wegovy® are typically mild to moderate, occur most often during dose escalation, and tend to improve over time. They may include: nausea, diarrhea, vomiting, constipation, abdominal pain, headache, and fatigue. More serious but less common risks include pancreatitis, gallbladder problems, kidney problems,, changes in vision, low blood sugar, and increased heart rate. Injectable Wegovy® should not be used in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Injection site reactions (redness, swelling, bruising) can occur but are generally mild and resolve quickly. Rotating injection sites each week minimizes this risk.

Has Wegovy® been studied for use in adolescents?

In adolescents aged 12-17 with obesity, Injectable Wegovy® produced a mean reduction in BMI of 16.1% compared to an increase of 0.6% with placebo over 68 weeks.

What has Wegovy® been approved for?

Injectable Wegovy® was approved by the FDA in 2021 for chronic weight management and expanded in 2024 to include cardiovascular risk reduction.

The clinical trials that demonstrated cardiovascular benefits (SELECT trial) and the landmark weight loss data (STEP trials) were all conducted with the injectable formulation, not the pill.
