General is not a medical group or a health care provider.General provides its users with the ability to obtain a telemedicine consultation or other health care services provided by independent medical practitioners including, but not limited to, Emerald Medical Practice, P.C. (AZ), Emerald Medical Practice, P.A. (KS), Emerald Medical Practice, P.C. (CA), Ruby Medical Practice, P.C. (NJ), Centennial Medical Practice, P.C. (ID) (each, a “Provider”, and collectively, the “Emerald Providers”).

This Privacy Notice does not apply to information collected from users who log-in to the password-protected and secure portions of our Website (“Secure Platform”). The Secure Platform allows users who obtain the Services (“Customers”) to perform certain functions or obtain the Services (such as telehealth visits) from providers, including Emerald Providers. All information collected and stored by us or added by Customers into such Secure Platforms is considered Protected Health Information ("PHI") and/or medical information and is governed by applicable state and federal laws that apply to that information, for example the Health Insurance Portability and Accountability Act (“HIPAA”). We will not use or disclose information collected from the Secure Platform or your provider for advertising or marketing except as otherwise permitted by HIPAA and other applicable law. We will not sell any PHI in violation of HIPAA. In addition, when PHI is collected or used by Emerald Providers (or is collected or received by General as a service provider to Emerald Providers), it is governed by the Emerald Providers’ HIPAA Notice of Privacy Practices and not this Privacy Notice. Emerald Providers (or your own medical provider, if you do not use an Emerald Provider) are responsible for providing you with a Notice of Privacy Practices describing its collection and use of your health information. Please see the Emerald Providers’ HIPAA Notice of Privacy Practices for more information on HIPAA, PHI, and your rights with respect to your PHI. To the extent there are inconsistencies between the Emerald Providers’ HIPAA Notice of Privacy Practices and this Privacy Notice, the HIPAA Notice of Privacy Practices will govern with respect to PHI.
OUR PRIVACY NOTICE AND TERMS OF SERVICE
This Privacy Notice is incorporated into our Terms of Service, which also apply when you use our Website.
PERSONAL INFORMATION WE COLLECT
As used in this Privacy Notice, “Personal Information” includes any information that can be used to personally identify you.We may collect and use the following Personal Information:
- Contact Information and identification details, such as your first and last name, email, mailing addresses, and phone number.
- Information and documents regarding identity, such as your driver’s license number.

- Health information, which may include details of your interaction with Emerald Providers and their responses, suggestions or guidance. As noted above, any PHI will be collected, used and disclosed in accordance with the Business Associate Agreements that General has in place with Emerald Providers, as well as the Emerald Providers’ Notice of Privacy Practices.
- Internet or other electronic network activity information, such as your IP address, device mode, device ID, OS version, device language, operating system, browser type, Website history, Website search history, and information regarding a consumer’s interaction with Website.
- Subscription and payment information including your purchases, transactions, billing and insurance information. We use a third-party payment processor to manage your payments, and we do not have access to your full payment card information.
HOW DO WE COLLECT YOUR PERSONAL INFORMATION?
Information you give us.We collect most of this Personal Information directly from you by receiving and storing information you provide to us in relation to our Services.For example, we collect Personal Information from you when you access the Services (for example, on our Website), complete electronic forms, complete the sign-up process, create an account, and when we communicate with you by phone, text message, email and through your electronic dashboard.
From third parties. We may obtain your Personal Information from a number of third parties, including marketing partners, data providers and providers using publicly-available sources.
We may also collect Personal Information from you in the following ways:

SOCIAL MEDIA INFORMATION
We may maintain pages on social media platforms, such as LinkedIn, X, and Instagram. When you visit or interact with our pages on those platforms, the platform provider’s privacy policy will apply to your interactions and their collection, use and processing of your personal information.
CALIFORNIA SHINE THE LIGHT DISCLOSURE
The California “Shine the Light” law gives residents of California the right under certain circumstances to request information from us regarding the manner in which we disclose certain categories of Personal Information with third parties for their direct marketing purposes.We do not disclose your Personal Information to third parties for their own direct marketing purposes.
