What is Auvelity?

Auvelity is a prescription combination medication approved for major depressive disorder in adults, pairing dextromethorphan (DXM)—an NMDA glutamate receptor antagonist—with bupropion, which both enhances DXM's CNS activity and provides its own antidepressant effects. The combination represents a new mechanism for treating depression: targeting the glutamate system rather than solely focusing on serotonin, dopamine, or norepinephrine. Bupropion inhibits the enzyme that rapidly breaks down DXM, allowing it to reach therapeutic brain levels for antidepressant effect. Auvelity is taken twice daily and demonstrated faster onset of action in clinical trials—with some patients experiencing improvement within one week.

NMDA receptor antagonism

Dextromethorphan blocks NMDA glutamate receptors in the brain, modulating excitatory neurotransmission and promoting synaptic plasticity—a depression treatment mechanism distinct from all existing monoamine-based antidepressants.

Bupropion as pharmacokinetic enhancer

Bupropion inhibits CYP2D6, the enzyme that rapidly metabolizes DXM, dramatically extending its half-life and allowing DXM to reach brain concentrations sufficient for antidepressant effect—while also contributing its own dopamine and norepinephrine reuptake inhibition.
