What is Zyvox (Linezolid)?

Zyvox is a prescription oxazolidinone antibiotic used to treat serious infections caused by gram-positive bacteria, including MRSA (methicillin-resistant Staphylococcus aureus), vancomycin-resistant Enterococcus (VRE), and drug-resistant pneumococcal pneumonia. It's one of the few oral antibiotics that achieves blood levels comparable to IV administration, making it a rare option for outpatient treatment of serious resistant infections. Linezolid works through a unique mechanism distinct from all other antibiotic classes, meaning bacteria resistant to other drugs rarely show pre-existing resistance to it. Duration of use is typically limited due to the risk of bone marrow suppression.

Unique 50S ribosomal binding

Binds to the 23S ribosomal RNA of the 50S subunit at a site distinct from all other antibiotics, blocking the formation of the ribosomal initiation complex and preventing the start of bacterial protein synthesis—a mechanism that bypasses existing antibiotic resistance.

MRSA and VRE activity

Effective against methicillin-resistant Staphylococcus aureus and vancomycin-resistant Enterococcus—two of the most difficult-to-treat resistant pathogens—providing treatment options when most other antibiotics have failed.

Oral bioequivalence to IV

Linezolid's oral formulation achieves blood levels nearly identical to IV administration, enabling treatment of serious drug-resistant infections in outpatient settings and facilitating hospital-to-home transitions without compromising therapeutic efficacy.

Frequently Asked Questions

What bacteria does Zyvox treat that other antibiotics cannot?

Zyvox is active against MRSA (methicillin-resistant Staphylococcus aureus) and vancomycin-resistant Enterococcus — two of the most difficult-to-treat drug-resistant bacteria — when many other antibiotics have failed.

Is there an oral form of Zyvox with the same effectiveness as IV?

Yes — one of Zyvox's most clinically important features is that its oral formulation achieves blood levels nearly identical to IV administration. This allows treatment of serious resistant infections without hospitalization in appropriate patients.

How long can I take Zyvox safely?

Most courses last 10–28 days. Longer use increases the risk of serious side effects including bone marrow suppression (thrombocytopenia) and serotonin syndrome when combined with serotonergic medications. Regular blood counts are needed.

What side effects might I experience?

Nausea, diarrhea, headache, and insomnia are most common. More serious effects include low platelet counts (thrombocytopenia), peripheral neuropathy with extended use, and serotonin syndrome risk with certain antidepressants.

Can I eat tyramine-containing foods while on Zyvox?

Limit tyramine-rich foods — aged cheeses, cured meats, fermented foods, and certain wines. Linezolid has weak MAO inhibitor activity, and consuming large amounts of tyramine can cause dangerous blood pressure elevations.
