What is Zetia (ezetimibe)?

Zetia is a prescription cholesterol-lowering medication that works differently from statins—rather than reducing cholesterol production in the liver, it blocks the absorption of cholesterol in the small intestine. It's used to lower LDL cholesterol either alone or in combination with a statin, where its complementary mechanism can achieve greater LDL reduction than either drug alone. Ezetimibe is taken once daily and is particularly valuable for patients who can't tolerate statins at higher doses or need additional LDL lowering beyond what their statin achieves. It's well-tolerated with a minimal side effect profile.

Intestinal cholesterol absorption blockade

Blocks the NPC1L1 transporter in the small intestinal lining, selectively preventing the absorption of dietary and biliary cholesterol without affecting the absorption of fat-soluble vitamins or triglycerides.

Complementary statin combination

Works through a completely different mechanism than statins, making the combination of Zetia and a statin more effective than either alone—statins reduce cholesterol production while Zetia reduces absorption, attacking LDL elevation from two independent directions.

Statin-intolerant alternative

For patients who can't tolerate full statin doses due to muscle side effects, Zetia provides meaningful LDL lowering through a mechanism unrelated to HMG-CoA reductase, offering a well-tolerated option to improve cholesterol control.

Frequently Asked Questions

How does Zetia lower cholesterol differently from statins?

Statins reduce cholesterol production in the liver. Zetia blocks cholesterol absorption in the small intestine — an entirely different mechanism. Combining the two attacks LDL from two independent directions for greater reduction.

How much can Zetia lower my LDL?

Zetia typically reduces LDL by 15–20% when used alone. When added to a statin, the combination achieves 20–25% additional LDL reduction beyond what the statin alone provides.

Does Zetia reduce cardiovascular events like statins do?

The IMPROVE-IT trial showed that adding Zetia to a statin after a heart attack modestly reduced cardiovascular events compared to the statin alone — confirming that additional LDL lowering through this mechanism does provide clinical benefit.

What side effects might I experience?

Zetia is generally very well tolerated. Diarrhea, joint pain, and upper respiratory infections are the most commonly reported effects. Unlike statins, Zetia does not cause the muscle-related side effects that lead many people to stop statin therapy.

Can Zetia be taken at any time of day?

Yes. Zetia can be taken once daily at any consistent time — with or without food. There are no specific timing requirements relative to meals, making it easy to incorporate into any daily routine.
