What is Zanaflex (Tizanidine)?

Zanaflex is a short-acting muscle relaxant prescribed to treat spasticity—the muscle stiffness, spasms, and increased muscle tone associated with conditions like multiple sclerosis and spinal cord injury. It works in the spinal cord by activating alpha-2 adrenergic receptors, which reduces the release of excitatory neurotransmitters that drive muscle overactivity. Because its effects last only 3–6 hours, it's typically timed to provide relief during specific activities like physical therapy or sleep. It causes significant sedation and blood pressure lowering, so doses are carefully titrated.

Alpha-2 adrenergic agonism

Activates alpha-2 receptors in the spinal cord, reducing the release of excitatory amino acids like glutamate and aspartate that drive the hyperreflexia and excessive muscle tone behind spasticity.

Short-acting targeted relief

Its 3–6 hour duration allows dosing to be timed strategically—providing muscle relaxation during specific periods of activity, therapy, or rest without continuous sedation throughout the day.

Spinal cord and MS spasticity

Particularly effective for the neurologically driven spasticity of multiple sclerosis and spinal cord injury, where overactive spinal reflex arcs cause painful, disabling muscle stiffness and involuntary spasms.

Frequently Asked Questions

How quickly does Zanaflex work for muscle spasticity?

Tizanidine is a short-acting medication — it typically begins reducing muscle tone and spasm within 1–2 hours of a dose and lasts 3–6 hours, making it practical to time around specific activities.

Can I use Zanaflex as needed or do I have to take it regularly?

Zanaflex can be used on a scheduled basis or strategically timed before activities that worsen spasticity — like physical therapy or sleep. Your clinician will advise on the best approach for your specific situation.

Can I stop Zanaflex suddenly?

Stopping tizanidine abruptly — particularly at higher doses — can cause rebound hypertension, tachycardia, and increased spasticity. Always taper gradually under your clinician's guidance when discontinuing.

What side effects might I experience?

Drowsiness, dizziness, weakness, dry mouth, and low blood pressure are the most common effects. Liver toxicity has occurred rarely — baseline liver function testing may be recommended before starting.

Does Zanaflex interact with other medications?

Yes — significant interactions exist with fluvoxamine and ciprofloxacin, which can dramatically increase tizanidine levels and cause dangerous low blood pressure. Always inform your clinician of all medications before starting.
