What is Xofluza (Baloxavir marboxil)?

Xofluza is an antiviral medication that treats influenza A and B in people who have been symptomatic for 48 hours or less. What makes it distinct from other flu treatments like Tamiflu is how it works—rather than blocking the virus from spreading between cells, Xofluza attacks an entirely different step in the viral replication process. It's taken as a single oral dose, making it one of the simplest antiviral regimens available. When taken early, it can shorten the duration of flu symptoms and reduce the risk of spreading the virus to others.

Cap-dependent endonuclease inhibition

Blocks a viral enzyme called cap-dependent endonuclease, preventing influenza from transcribing its genetic material inside host cells—an entirely different mechanism from other flu antivirals.

Single-dose treatment

The entire treatment course is one oral dose, eliminating the 5-day twice-daily regimen required by older antivirals and making it significantly easier to complete treatment correctly.

Viral load reduction

By halting viral replication at an early stage, Xofluza rapidly reduces the amount of virus in the body—shortening symptom duration and potentially lowering the risk of transmitting flu to close contacts.

Frequently Asked Questions

How is Xofluza different from Tamiflu?

Xofluza works through a completely different mechanism than Tamiflu and requires only a single oral dose versus Tamiflu's 5-day twice-daily regimen. It may also reduce the period of viral shedding more rapidly.

Is one dose really enough?

Yes — one weight-based dose is the complete treatment for most patients. This removes the risk of forgetting follow-up doses and makes completing treatment significantly simpler than multi-day regimens.

How quickly does it work?

Like other flu antivirals, Xofluza works best when taken within 48 hours of symptom onset. Symptom duration is typically shortened by 1–2 days when started early in the illness.

What side effects might I experience?

Xofluza is generally well-tolerated. Diarrhea, nausea, headache, and bronchitis have been reported. Side effects are typically mild given the single-dose format and the short duration of drug exposure.

Can Xofluza be used in children?

Yes. Xofluza is approved for treatment of influenza in patients 5 years and older who have been symptomatic for less than 48 hours. Dosing is weight-based — your clinician will determine the appropriate amount.
