What is Xenical (orlistat)?

Xenical is a prescription weight loss medication that works by blocking the digestion and absorption of dietary fat in the gastrointestinal tract. Unlike centrally acting weight loss drugs that suppress appetite or affect the brain, Orlistat works entirely within the gut—inhibiting pancreatic lipase enzymes so that about one-third of the fat consumed in a meal passes through the digestive system unabsorbed. It's taken with each main meal containing fat and is used alongside a reduced-calorie diet and exercise. The unabsorbed fat that passes through can cause oily or loose stools, particularly if a high-fat meal is eaten.

Pancreatic lipase inhibition

Blocks pancreatic and gastric lipase enzymes that break down dietary triglycerides into absorbable fatty acids and monoglycerides—preventing approximately 30% of consumed fat from being digested and absorbed.

Gut-localized mechanism

Works entirely within the gastrointestinal tract without entering the bloodstream in significant amounts, avoiding the cardiovascular and CNS side effects associated with systemic weight loss medications.

Dietary fat consequence

Unabsorbed fat passes into the large intestine and is excreted, which reinforces reduced fat intake—the oily, loose stools that can result from high-fat meals serve as a practical behavioral feedback mechanism encouraging adherence to a lower-fat diet.

Frequently Asked Questions

How much fat does Xenical actually block?

Xenical blocks the digestion and absorption of approximately one-third of the fat consumed in a meal. The unabsorbed fat passes through the intestine and is excreted in the stool rather than being stored.

Do I need to follow a specific diet while on Xenical?

Yes. A low-fat diet — with no more than 30% of calories from fat — is essential. High-fat meals significantly worsen GI side effects and undermine the weight loss benefit. Dividing fat evenly across meals also helps.

How much weight loss can I expect from Xenical?

Clinical trials showed approximately 5–7% body weight loss over one year alongside diet and exercise — modest but clinically meaningful when sustained. Xenical is a tool, not a standalone solution.

What side effects might I experience?

Oily or fatty stools, urgent need to defecate, oily spotting, gas with discharge, and fecal incontinence are the most distinctive side effects — directly caused by the unabsorbed fat. These are worst when high-fat meals are eaten.

Does Xenical affect absorption of vitamins?

Yes. Because it reduces fat absorption, Xenical can also reduce absorption of fat-soluble vitamins A, D, E, and K. A daily multivitamin containing these vitamins should be taken — at least 2 hours before or after Xenical.
