What causes vaginal dryness?

The most common cause is declining estrogen — during perimenopause, menopause, postpartum, and breastfeeding. Certain medications — antihistamines, antidepressants, hormonal contraceptives in some women — can also reduce vaginal lubrication. Sjögren's syndrome and other conditions are less common causes.

Is vaginal dryness just a cosmetic issue or does it affect health?

Vaginal dryness is part of a broader condition called genitourinary syndrome of menopause (GSM) — which also affects the bladder and urethra. It can cause painful intercourse, recurrent UTIs, urinary urgency, and significantly impact quality of life and sexual health.

Is vaginal estrogen safe?

Local vaginal estrogen — creams, rings, suppositories — delivers very low doses with minimal systemic absorption and is considered safe for most women, including many breast cancer survivors. It is not the same as systemic hormone therapy and does not carry the same risk profile.

Are there non-hormonal options?

Yes — vaginal moisturizers (used regularly, not just before intercourse) and lubricants (used during intercourse) provide meaningful relief for many women. Ospemifene — an oral SERM — is an FDA-approved non-estrogen prescription option for dyspareunia caused by vaginal atrophy.

What side effects are associated with vaginal dryness treatments?

Local vaginal estrogen is very well tolerated — mild initial irritation may occur. Ospemifene has a small risk of hot flash worsening and carries typical SERM precautions.
