What is Urso (Ursodiol)?

Urso is a prescription bile acid medication used to treat primary biliary cholangitis (PBC)—a chronic liver disease where the immune system attacks the bile ducts—and to dissolve certain types of gallstones in patients who are not surgical candidates. Ursodiol is a naturally occurring bile acid that, when taken as a medication, improves bile composition, protects liver cells from the toxic effects of accumulated bile, and has anti-inflammatory and immune-modulating properties in the liver. In PBC, it slows disease progression and improves liver function tests. It's taken two to four times daily with food.

Bile acid pool modification

Replaces toxic hydrophobic bile acids with the more hydrophilic and cytoprotective ursodiol, reducing the detergent-like cellular damage that concentrated bile causes to liver cells and bile duct epithelium in cholestatic liver disease.

Hepatoprotective and anti-inflammatory

Protects hepatocytes from bile acid-induced apoptosis and membrane damage, while also modulating immune cell activity in the liver—slowing the immune-driven bile duct destruction that characterizes primary biliary cholangitis.

Gallstone dissolution

In patients with cholesterol-rich gallstones who cannot undergo surgery, Ursodiol gradually reduces biliary cholesterol saturation and allows cholesterol gallstones to slowly dissolve over months to years of treatment.

Frequently Asked Questions

What conditions does Urso treat?

Urso is used to treat primary biliary cholangitis (PBC) — a chronic liver condition — by slowing disease progression and improving liver function. At different doses, it's also used to gradually dissolve certain cholesterol gallstones in surgical candidates.

How long before Urso improves liver function tests in PBC?

Liver enzyme improvements — particularly ALP and bilirubin — are typically seen within the first 3–6 months of consistent dosing. Long-term use slows disease progression, with benefit continuing over years of treatment.

How long does gallstone dissolution with Urso take?

Dissolving cholesterol gallstones is a slow process — typically requiring 6–24 months of treatment. Stones may recur after stopping, and many clinicians continue treatment for prevention in appropriate patients.

What side effects might I experience?

Diarrhea, constipation, nausea, and stomach upset are the most commonly reported effects. Urso is generally very well tolerated — its safety profile is a significant advantage for a condition requiring long-term daily treatment.

Can Urso be used during pregnancy?

Urso is sometimes used during pregnancy for intrahepatic cholestasis of pregnancy — a condition causing intense itching — under careful clinician supervision. The decision requires individualized risk-benefit assessment.
