What type of urinary incontinence do I have?

The two most common types are stress incontinence — leakage with physical exertion, coughing, or sneezing due to pelvic floor weakness — and urge incontinence — sudden, strong urge followed by leakage due to bladder overactivity. Mixed incontinence involves both. Identifying the type guides treatment.

Are pelvic floor exercises effective?

Kegel exercises — consistent, correctly performed pelvic floor muscle training — are the most evidence-supported first-line treatment for stress incontinence and can also help with urge incontinence. Working with a pelvic floor physical therapist significantly improves outcomes compared to self-directed exercises.

Will I need surgery for incontinence?

Most women achieve meaningful improvement with conservative treatments — pelvic floor therapy, behavioral strategies, and medication. Surgery is reserved for stress incontinence that hasn't responded adequately to conservative care. Minimally invasive procedures are available with good long-term outcomes.

Does urinary incontinence always worsen with age?

Not necessarily. While pelvic floor changes with age and hormonal shifts contribute to incontinence, appropriate treatment can produce significant improvement at any age. Untreated incontinence does tend to progress — early evaluation and intervention produce the best outcomes.

What side effects are associated with incontinence treatments?

Pelvic floor therapy has no side effects. Anticholinergics for urge incontinence may cause dry mouth, constipation, and cognitive effects in older adults. Beta-3 agonists have fewer cognitive concerns. Topical vaginal estrogen is very well tolerated and helpful for postmenopausal women.
