What is Trulance?

Trulance is a prescription medication used to treat chronic idiopathic constipation and irritable bowel syndrome with constipation (IBS-C) in adults. It works locally in the upper gastrointestinal tract, mimicking a naturally occurring gut hormone called uroguanylin that activates fluid and electrolyte secretion into the intestine. This increases the fluid content of the bowel and accelerates transit, softening stool and making bowel movements more frequent and easier to pass. Like Linzess, it works within the gut without meaningful systemic absorption, and is taken once daily on an empty stomach.

Guanylate cyclase-C agonism

Binds to and activates GC-C receptors in intestinal epithelial cells, stimulating cAMP and cGMP production that drives chloride and bicarbonate secretion into the gut lumen—increasing intestinal fluid and accelerating colonic transit.

pH-dependent upper GI activation

Trulance is uniquely designed to activate primarily in the slightly acidic environment of the upper small intestine, closely mimicking the site and conditions under which natural uroguanylin acts—a physiologically targeted approach.

Local gut-only action

Minimal systemic absorption means Trulance's entire therapeutic effect is confined to the GI tract, reducing the potential for systemic side effects while concentrating its fluid-secreting and motility-enhancing activity exactly where it's needed.
