What is a Triptan?

Triptans are a class of prescription medications used to treat acute migraine and cluster headache attacks—they stop an attack in progress rather than preventing future ones. They all work through the same core mechanism: activating serotonin 5-HT1B and 5-HT1D receptors in the brain to constrict dilated cranial blood vessels and block the release of inflammatory neuropeptides from trigeminal nerve endings. Different triptans vary in their speed of onset, duration of action, available formulations, and tolerability—allowing providers to find the best fit for individual patients. They're most effective when taken at the first sign of a migraine.

Serotonin receptor agonism

All triptans activate 5-HT1B receptors on cranial blood vessels to reverse the vasodilation associated with migraines, and 5-HT1D receptors on trigeminal nerve terminals to block the release of pain-promoting neuropeptides like CGRP and substance P.

Triptans are most effective when taken as early as possible during an attack—early dosing interrupts the migraine cascade before central sensitization is established, resulting in faster and more complete pain relief.

What is the difference between a triptan and a pain reliever for migraines?

Triptans work by targeting the specific neurological and vascular processes that cause migraine — not just masking pain. They constrict dilated blood vessels and block the inflammatory neuropeptides driving the attack.

Which triptan is most effective?

There is no single best triptan — response is highly individual. What works well for one person may not for another. If the first triptan tried doesn't provide adequate relief, trying a different one or a different formulation is appropriate.

Can I take a triptan and an OTC pain reliever together for a migraine?

Some combinations are safe — taking a triptan alongside naproxen sodium, for example, is supported by clinical evidence. Avoid combining triptans with each other or with ergotamine-type medications within 24 hours.

What side effects might I experience from triptans?

Tingling, flushing, dizziness, drowsiness, and sensations of tightness or pressure in the chest, neck, or throat are common across the triptan class. These are usually mild and brief. Chest symptoms should be evaluated if they're severe or new.

Can I take triptans if I have migraines with aura?

Yes — triptans are approved for migraine with or without aura. However, they should not be taken during the aura phase itself, only when headache begins. Triptans are contraindicated in hemiplegic or basilar migraine.
