What is Trexall (Methotrexate)?

Trexall is a prescription medication used at low doses to treat autoimmune conditions including rheumatoid arthritis, psoriatic arthritis, and psoriasis, and at higher doses as a chemotherapy agent for certain cancers. For autoimmune disease, it works by suppressing the overactive immune response that attacks the body's own tissues—reducing joint inflammation, skin plaques, and long-term structural damage. It's taken once weekly (not daily) as a tablet, and requires regular blood monitoring to watch for effects on the liver and blood cell counts. Folic acid is typically taken alongside it to reduce side effects.

Folate pathway inhibition

Blocks dihydrofolate reductase, impairing DNA synthesis and cell replication—at low doses this selectively suppresses rapidly proliferating immune cells driving autoimmune inflammation; at higher doses it targets cancer cells.

Disease-modifying anti-inflammatory effect

Reduces the production of inflammatory cytokines and suppresses the immune cell activity responsible for joint destruction in rheumatoid arthritis and skin inflammation in psoriasis—slowing disease progression over time.

Why do I need to take folic acid with Trexall?
