What is Trental (Pentoxifylline)?

Trental is a prescription medication used to improve blood flow in patients with intermittent claudication—the leg pain, cramping, and fatigue that occur during walking due to peripheral artery disease. It belongs to a class called xanthine derivatives and works by improving the flexibility of red blood cells, reducing blood viscosity, and decreasing platelet aggregation—all of which allow blood to flow more easily through narrowed peripheral blood vessels. It doesn't open blocked arteries but improves circulation within the existing vessel structure. Taken three times daily with meals, it helps patients walk farther before symptoms appear.

Red blood cell deformability improvement

Increases erythrocyte flexibility, allowing red blood cells to pass more easily through narrowed, partially obstructed peripheral blood vessels—improving oxygen delivery to ischemic muscle tissue during exercise.

Blood viscosity reduction

Decreases plasma viscosity and platelet aggregation, reducing the resistance to blood flow in peripheral vessels and improving overall microcirculatory perfusion in affected limbs.

Claudication symptom relief

By optimizing blood flow through existing vessels rather than opening blocked ones, Pentoxifylline extends the distance patients with peripheral artery disease can walk before cramping and pain force them to stop—improving functional capacity and quality of life.

Frequently Asked Questions

What is Trental used for?

Trental improves blood flow in peripheral artery disease — specifically for intermittent claudication, where narrowed arteries cause leg pain, cramping, and fatigue during walking that forces people to stop and rest.

How quickly does Trental improve walking distance?

Improvement in pain-free walking distance is gradual — most studies show meaningful benefit developing over 4–8 weeks of consistent three-times-daily use. Some patients see continued improvement over 2–3 months.

Does Trental cure peripheral artery disease?

No. Pentoxifylline improves blood flow through existing vessels but does not open blocked arteries or treat the underlying atherosclerosis. Lifestyle changes, cholesterol management, and cardiovascular risk reduction remain essential.

What side effects might I experience?

Nausea, vomiting, dizziness, and headache are the most commonly reported effects. Taking Trental with meals significantly reduces GI side effects. Flushing can also occur. Serious side effects are uncommon at therapeutic doses.

Does Trental interact with other medications?

Yes — particularly with blood thinners like warfarin, aspirin, and NSAIDs, which may increase bleeding risk when combined. Blood pressure medications may also be affected. Inform your clinician of all medications before starting.
