What are the most common causes of tongue sores or irritation?

Canker sores (aphthous ulcers), geographic tongue (benign inflammatory condition causing map-like patches), burning mouth syndrome, trauma from sharp teeth or dental work, oral thrush (candidiasis), and viral infections (herpes, hand-foot-and-mouth disease) are the most frequent causes of tongue symptoms.

When should a tongue sore be evaluated by a clinician?

Seek evaluation for any sore persisting beyond 2–3 weeks, ulcers with raised or indurated (hardened) borders, unexplained white or red patches that don't resolve, tongue asymmetry or unexplained pain without a visible lesion, or any sore in a patient with tobacco or alcohol use — to rule out oral cancer.

Could my tongue irritation be caused by a nutritional deficiency?

Yes — deficiencies in B12, iron, folate, and zinc can cause glossitis (tongue inflammation), burning, and soreness. Glossitis from deficiency often presents as a smooth, beefy red tongue with loss of normal papillae. Blood testing for these deficiencies is an important part of evaluation for persistent, unexplained tongue symptoms.

Can certain foods or products cause tongue irritation?

Yes — acidic foods, spicy foods, and SLS (sodium lauryl sulfate) in some toothpastes are common irritants. Food allergies can cause oral allergy syndrome — itching and tingling of the tongue and mouth after exposure to specific raw foods. Identifying and avoiding triggers often provides significant relief.

What side effects are associated with tongue sore treatments?

Topical anesthetics (benzocaine) are well tolerated — use should be limited in young children. Topical corticosteroids may cause mild local thinning with prolonged use. Antifungals for oral thrush are very safe with topical use. Nutritional supplementation for identified deficiencies is safe at appropriate doses.
