What are the most common causes of testicular pain or discomfort?

Epididymitis (inflammation of the epididymis — most commonly from bacterial infection), varicocele (enlarged veins in the scrotum), hydrocele (fluid accumulation), inguinal hernia, orchitis, post-vasectomy pain, and — importantly — testicular torsion all cause testicular discomfort. Referred pain from the back or abdomen can also be felt in the testicle.

When is testicular pain a medical emergency?

Testicular torsion — twisting of the spermatic cord cutting off blood supply — is a urological emergency requiring surgical intervention within 6 hours to preserve the testicle. Signs include sudden severe testicular pain, nausea, vomiting, and an elevated or horizontally lying testicle. Any sudden severe testicular pain requires emergency evaluation.

Should every testicular lump be evaluated?

Yes — any new testicular lump, hardness, or asymmetry should be evaluated promptly, particularly in young men aged 15–35 who have the highest incidence of testicular cancer. Scrotal ultrasound is the first-line imaging and is highly accurate at distinguishing benign from malignant lesions.

How do I perform a testicular self-exam?

Monthly self-exams after a warm shower — when the scrotal skin is relaxed — allow early detection of changes. Gently roll each testicle between the thumb and fingers, feeling for new lumps, hardness, or size changes. The epididymis at the back of each testicle is normally bumpy — this is not a lump.

What side effects are associated with testicular discomfort treatments?

Antibiotics for epididymitis carry standard GI side effect considerations. NSAIDs for pain and inflammation are very effective with standard GI precautions. Surgical treatment for torsion or hernia carries procedural risks. Varicocele embolization or ligation is generally well tolerated with low complication rates.
