How do I know if my sore throat needs medical attention?

Most sore throats are viral and resolve without treatment. Seek evaluation for sore throat with high fever, difficulty swallowing or opening the mouth, drooling, voice change, swelling visible in the throat, rash, symptoms persisting beyond 5–7 days, or known strep exposure — these features suggest bacterial infection or another cause requiring treatment.

Does a sore throat always need a strep test?

Not always. Strep throat has characteristic features — sudden onset, fever, tonsillar exudate, absence of cough — that are scored with clinical tools to guide testing decisions. Clinical prediction rules help identify who benefits most from a rapid strep test versus who can be managed empirically.

Can a sore throat be caused by acid reflux?

Yes — laryngopharyngeal reflux can cause chronic throat irritation, soreness, and the sensation of a lump in the throat (globus) — often without classic heartburn. This is an underrecognized cause of persistent sore throat symptoms that doesn't resolve with rest or OTC remedies.

What can I do at home to relieve sore throat pain?

Saltwater gargles, warm liquids (broth, tea with honey), OTC throat sprays with benzocaine, lozenges, and systemic analgesics (ibuprofen, acetaminophen) provide meaningful symptom relief. Staying well hydrated supports recovery and reduces throat dryness.

What side effects are associated with sore throat treatments?

Antibiotics for confirmed strep may cause GI upset, diarrhea, and rash. Topical benzocaine sprays carry a rare risk of methemoglobinemia — not recommended for infants.
