How do I know if my shoulder needs surgery or physical therapy first?

The vast majority of shoulder problems — including partial rotator cuff tears, impingement, tendinopathy, and mild instability — respond well to physical therapy and do not require surgery. Surgical referral is appropriate when conservative management has been adequately tried without sufficient improvement, or when imaging shows complete structural disruption.

What does shoulder surgery recovery typically involve?

Recovery from rotator cuff surgery typically involves 4–6 weeks in a sling followed by progressive PT over 4–6 months before return to full activity. Labral repair recovery follows a similar timeline. Full recovery to pre-injury function can take 6–12 months. Adherence to the rehabilitation program is the most important determinant of outcome.

Will I need general anesthesia for shoulder surgery?

Most shoulder surgeries are performed arthroscopically (small incisions, minimally invasive) under general anesthesia combined with regional nerve block (interscalene block) — providing excellent intraoperative and post-operative pain control. Open procedures for complex repairs are less common.

What risks are associated with shoulder surgery?

Standard risks include infection, bleeding, nerve injury (particularly the axillary nerve), stiffness, incomplete healing of the repaired tissue, and anesthesia-related complications.
