What are the most common causes of a new acute cough?

Viral upper respiratory infections are by far the most common cause of acute cough — accounting for the majority of cases. Post-nasal drip from rhinitis or sinusitis, acute bronchitis, and new asthma or reactive airway disease are other frequent causes. Most acute coughs resolve within 3 weeks without specific treatment.

When does a new cough need clinical evaluation?

Seek evaluation for cough with high fever, shortness of breath, chest pain, blood-tinged sputum, significant weight loss, or cough persisting beyond 3 weeks without improvement. A new cough in a smoker, immunocompromised patient, or someone with known heart or lung disease also warrants earlier assessment.

Does a cough with colored mucus mean I need antibiotics?

Not necessarily. Yellow or green sputum reflects normal immune cell activity and can occur with viral infections — it does not reliably indicate bacterial infection. Antibiotics are not routinely indicated for acute cough from viral bronchitis or the common cold, regardless of mucus color.

What can I do to relieve a cough while waiting for it to resolve?

Staying hydrated, using a humidifier, honey (in adults and children over 1), elevating the head during sleep, and avoiding irritants like smoke can help. OTC cough suppressants (dextromethorphan) or expectorants (guaifenesin) provide modest symptomatic relief for some patients.

What side effects are associated with acute cough treatments?

Dextromethorphan may cause drowsiness and dizziness. Guaifenesin is very well tolerated. Antihistamine-decongestant combinations may cause sedation or elevated blood pressure. Codeine-containing cough syrups carry dependency risk and are not recommended as first-line. Unnecessary antibiotics drive resistance and GI side effects.
