Other warning signs include headache with fever and neck stiffness, new neurological symptoms, headache after head trauma, or headache in someone with cancer or immunosuppression.

How is a severe headache evaluated in a clinical setting?

Evaluation begins with a thorough history — onset, character, associated symptoms, prior headache history — and neurological exam. Imaging (CT, MRI) is performed when secondary causes are suspected based on red flag features. Most severe headaches in patients with a prior migraine history are primary — not structural.

Can a severe headache be a migraine even if I've never had migraines before?

Yes — migraine can present for the first time with a severe attack. However, a first-ever severe headache — particularly if sudden in onset — requires evaluation to rule out secondary causes before migraine is attributed as the cause.

What is the best acute treatment for a severe migraine?

Triptans remain a mainstay of acute migraine treatment. IV or IM treatments — including ketorolac, prochlorperazine, and valproate — are used in urgent care or emergency settings for severe, intractable migraines. Newer options including gepants are well tolerated and avoid cardiovascular contraindications.

What side effects are associated with treatments for severe headache?

Triptans may cause chest pressure, tingling, and flushing — generally brief. IV prochlorperazine may cause restlessness (akathisia). Ketorolac carries GI and renal side effect risks with repeated use. Gepants are well tolerated with a favorable cardiovascular and side effect profile.
