What is tinea capitis and who is most commonly affected?

Tinea capitis is a fungal infection of the scalp caused by dermatophytes — most commonly Trichophyton tonsurans in the US. It primarily affects children but can occur in adults, particularly those who are immunocompromised or have close contact with an infected child. It is contagious and common in school-age children.

What does tinea capitis look like?

Presentations vary — scaly, itchy patches with broken-off hairs are most common. Some cases cause significant inflammation (kerion) — a boggy, tender, pus-filled plaque that can lead to scarring and permanent hair loss if untreated. Black dot tinea capitis shows dark dots where hairs have broken at scalp level.

Why can't tinea capitis be treated with topical antifungal creams?

Tinea capitis invades the hair shaft and follicle — topical antifungals cannot penetrate deeply enough to eradicate the infection. Oral antifungal treatment (griseofulvin, terbinafine, fluconazole, or itraconazole) is required for effective treatment.

Do other household members need to be evaluated or treated?

Asymptomatic household contacts — particularly other children — should be screened, as they may be carriers without symptoms. Antifungal shampoo (selenium sulfide or ketoconazole) is often recommended for household contacts to reduce transmission, even without oral treatment.

What side effects are associated with tinea capitis treatments?

Griseofulvin is generally well tolerated — GI upset and photosensitivity are the most common effects. Terbinafine may cause GI symptoms and rarely liver enzyme changes. Treatment courses of 6–12 weeks are typically required. Sharing combs, hats, and pillowcases should be avoided during treatment.
