What is scalp folliculitis and what causes it?

Scalp folliculitis is inflammation of the hair follicles — presenting as small, itchy red bumps or pustules on the scalp, often mistaken for acne. It can be caused by bacterial infection (most commonly Staphylococcus aureus), fungal infection (Malassezia), or be non-infectious — related to product buildup, occlusion, or seborrheic dermatitis.

How do I know if my scalp folliculitis is bacterial or fungal?

Bacterial folliculitis typically presents as discrete pustules with possible crusting. Fungal (Malassezia) folliculitis tends to appear as uniform small papules and pustules, often more diffuse — and may not respond to antibiotics. A clinical evaluation — sometimes with culture or KOH preparation — distinguishes between types.

Can the products I use on my hair cause scalp folliculitis?

Yes — occlusive hair products (pomades, heavy conditioners, oils) can block follicle openings, creating conditions for bacterial or fungal overgrowth. Tight hairstyles causing repeated friction and occlusion are also contributing factors. Simplifying hair care and avoiding heavy products often improves symptoms.

Will scalp folliculitis cause permanent hair loss?

Most folliculitis resolves without permanent hair loss once the infection is treated. Severe, deep, or recurrent folliculitis — particularly decalvans (a scarring form) — can cause permanent follicle destruction. Prompt treatment of acute folliculitis reduces this risk.

What side effects are associated with scalp folliculitis treatments?

Topical antibiotics (clindamycin solution) are well tolerated. Antifungal shampoos (ketoconazole, selenium sulfide) may cause mild dryness or irritation. Oral antibiotics carry standard GI and resistance considerations. Oral antifungals require liver function monitoring with prolonged use. Medicated shampoos used preventively are very safe long-term.
