What is Quinidine?

Quinidine is an antiarrhythmic medication used to restore and maintain normal heart rhythm in certain types of atrial fibrillation, atrial flutter, and ventricular arrhythmias. It's one of the oldest antiarrhythmic drugs available, derived from cinchona bark, and works by stabilizing the electrical activity of heart muscle cells. It's also used in combination with certain malaria treatments and has a rare but important use in treating a dangerous inherited heart rhythm disorder called Brugada syndrome. Because of its narrow therapeutic window, it requires careful monitoring.

Sodium channel blockade

Blocks fast sodium channels in cardiac muscle cells, slowing the rate of electrical depolarization and reducing the abnormal rapid firing that causes arrhythmias—stabilizing the heart's rhythm at the cellular level.

Atrial arrhythmia control

Slows conduction through the atria and AV node, helping convert and maintain normal sinus rhythm in atrial fibrillation and flutter by interrupting the re-entrant electrical circuits that sustain these rhythms.

Narrow therapeutic window

Quinidine levels must be carefully monitored because the difference between a therapeutic dose and a toxic one is small—toxicity can cause its own dangerous arrhythmias, making regular monitoring and dose adjustment essential.
