What is Questran (cholestyramine)?

Questran is a bile acid sequestrant used to lower LDL cholesterol and to treat itching caused by bile acid accumulation in liver disease. It works in the intestine by binding to bile acids—which are made from cholesterol—and preventing them from being reabsorbed. The body then has to draw on cholesterol stores to make new bile acids, reducing circulating LDL levels. It's taken as a powder mixed with water or juice, one to six times daily depending on the dose, and doesn't enter the bloodstream—making it one of the safest cholesterol medications for certain populations, including pregnant women.

Bile acid sequestration

Binds bile acids in the intestinal lumen and prevents their reabsorption into the enterohepatic circulation, forcing the liver to synthesize new bile acids from circulating cholesterol—thereby reducing LDL cholesterol levels.

Non-systemic mechanism

Cholestyramine is not absorbed into the bloodstream and works entirely within the GI tract—making it one of the few cholesterol-lowering medications considered safe during pregnancy and for patients who cannot tolerate systemic drugs.

How do I take Questran powder?

Mix the powder thoroughly in 4–6 ounces of water, juice, or another non-carbonated beverage until fully dissolved. Take it with meals. Never take it dry — the undissolved powder can cause choking or esophageal irritation.

How long before Questran lowers my cholesterol?

LDL-lowering effects are typically measurable within 4–6 weeks. Your clinician will check a lipid panel to assess response and determine whether dose adjustment or additional therapy is needed.

Why does Questran cause constipation?

Cholestyramine binds bile acids in the intestine and also has constipating properties. Increasing fluid intake, dietary fiber, and physical activity helps significantly. Your clinician may recommend a stool softener if needed.

What side effects might I experience?

Constipation, bloating, gas, nausea, and stomach discomfort are the most common — particularly when starting. These often improve after the first few weeks. High doses may also reduce absorption of fat-soluble vitamins A, D, E, and K.

Does Questran interact with other medications?

Yes — significantly. Questran can bind other oral medications and reduce their absorption. Take all other medications at least 1 hour before or 4–6 hours after Questran. This includes statins, thyroid medication, and warfarin.
