What is a pulmonary nodule and should I be worried?

A pulmonary nodule is a small, rounded area of abnormal tissue in the lung — most found incidentally on CT scans performed for other reasons. The vast majority are benign — caused by prior infection (histoplasmosis, granuloma), scar tissue, or other non-cancerous processes. Malignancy risk depends on size, characteristics, and patient risk factors.

How is a pulmonary nodule evaluated?

Management follows established guidelines (Fleischner Society or Lung-RADS) based on nodule size, density (solid vs. ground glass vs. part-solid), growth rate, and patient risk factors (smoking history, age, prior cancer). Most small, stable nodules require only interval CT surveillance — biopsy or PET scan is reserved for higher-risk features.

How long will I need to follow up with imaging?

Follow-up duration depends on nodule characteristics and risk profile. Most small, low-risk nodules require 1–2 follow-up CT scans over 1–2 years to confirm stability — after which surveillance can often be discontinued. Larger or higher-risk nodules may require longer follow-up or more active evaluation.

What would prompt a biopsy of a pulmonary nodule?

Biopsy is considered when a nodule has grown on serial imaging, has high-risk characteristics (irregular margins, new ground-glass component, PET-avid), or is large enough that management would change with a tissue diagnosis. The biopsy approach — bronchoscopic, CT-guided percutaneous, or surgical — depends on nodule location and size.

Are there risks associated with pulmonary nodule evaluation?

CT scans involve low-level radiation — the benefit of surveillance typically far exceeds this risk. CT-guided biopsy carries a risk of pneumothorax (approximately 15–20%) — most resolve without treatment. PET scanning involves radiation from the radiotracer.
