What is pityriasis rosea and what causes it?

Pityriasis rosea is a self-limiting skin condition causing a distinctive pattern of oval, salmon-colored, scaly patches — typically beginning with a single 'herald patch' followed 1–2 weeks later by a widespread rash in a 'Christmas tree' distribution on the trunk. It's thought to be triggered by reactivation of human herpesvirus 6 or 7.

Is pityriasis rosea contagious?

Pityriasis rosea is not considered meaningfully contagious — there is no evidence of person-to-person spread through casual contact. Clustering of cases in households is occasionally reported but is thought to reflect shared viral exposure rather than direct transmission.

How long does pityriasis rosea last?

Most cases resolve spontaneously within 6–10 weeks without treatment. The rash fades gradually — residual temporary hyperpigmentation or hypopigmentation is common, particularly in darker skin tones, and may take months to fully normalize. Recurrence is uncommon.

Is treatment necessary for pityriasis rosea?

Treatment is not required — most cases resolve on their own. For significant itching, oral antihistamines, topical corticosteroids, and moisturizers provide symptomatic relief. For widespread or prolonged cases, acyclovir (antiviral) has limited supporting evidence. Avoiding hot showers can reduce itching.

What side effects are associated with pityriasis rosea treatments?

Topical corticosteroids for itching relief are well tolerated for short-term use. Oral antihistamines may cause drowsiness — second-generation options (cetirizine, loratadine) have less sedating effect. Acyclovir is generally well tolerated — nausea and headache are the most common effects. Adequate hydration reduces acyclovir-related kidney concerns.
