What are pinworms and how are they transmitted?

Pinworms (Enterobius vermicularis) are small intestinal parasites that cause intense perianal itching — typically worse at night, when female worms emerge to lay eggs on the skin. They spread through the fecal-oral route — most commonly hand-to-mouth after touching contaminated surfaces. They're highly prevalent in school-age children.

How is a pinworm infection diagnosed?

The scotch tape test — pressing clear tape to the perianal skin first thing in the morning before bathing — is the standard diagnostic method. The tape is then examined under a microscope for eggs. Multiple tests on different mornings may be needed as sensitivity per test is imperfect.

Do all household members need to be treated?

Yes — simultaneous treatment of all household members is strongly recommended, regardless of symptoms. Pinworms spread easily among household contacts, and treating only the symptomatic individual almost always results in reinfection.

How do I prevent reinfection after treatment?

Thorough handwashing — especially after toileting and before eating — nail trimming, daily morning showering (to remove eggs deposited overnight), and washing bedding and underwear in hot water after treatment are the key prevention measures. A second treatment dose 2 weeks after the first is standard practice.

What side effects are associated with pinworm treatments?

Mebendazole and albendazole — the most commonly used treatments — are very well tolerated at the single doses used for pinworm. Mild GI upset (nausea, abdominal discomfort) is occasionally reported.
