Frequently Asked Questions

What causes painful intercourse?

Common causes include vaginal dryness (from menopause, breastfeeding, or hormonal contraceptives), vaginismus (involuntary pelvic floor muscle spasm), vulvodynia, endometriosis, pelvic inflammatory disease, ovarian cysts, interstitial cystitis, and insufficient arousal. Identifying the specific cause is essential — treatments differ widely.

Is painful intercourse normal or something I should have evaluated?

Painful intercourse is never something to simply accept. It significantly impacts quality of life and relationships and is frequently undertreated because many women feel embarrassed to raise it. A thorough clinical evaluation can identify the cause in most cases and guide appropriate, effective treatment.

How does pelvic floor physical therapy help with dyspareunia?

For dyspareunia caused by pelvic floor muscle tension, vaginismus, or post-surgical changes, pelvic PT is a highly effective first-line treatment. A specialized therapist assesses and treats muscle function, coordination, and sensitivity through manual techniques and progressive desensitization exercises.

Can hormonal treatment help with painful intercourse?

Yes — for dyspareunia caused by vaginal atrophy or dryness (particularly in menopausal or postpartum women), local vaginal estrogen, ospemifene, or DHEA (Intrarosa) can produce significant improvement. These are distinct from systemic HRT and are safe for most women.

What side effects are associated with dyspareunia treatments?

Vaginal estrogen is very well tolerated with minimal systemic absorption. Ospemifene may cause hot flashes. Pelvic PT has no physical side effects — some temporary soreness after sessions is normal. Dilator therapy requires patience and gradual progression. Treating endometriosis or other structural causes involves their respective treatment considerations.
