What is Otezla (Apremilast)?

Otezla is a prescription oral medication used to treat moderate to severe plaque psoriasis, active psoriatic arthritis, and oral ulcers associated with Behçet's disease. Unlike biologic medications that require injection, Otezla is taken as a pill twice daily and works by inhibiting PDE4—an enzyme inside immune cells that regulates the production of inflammatory cytokines. By blocking PDE4, it raises cyclic AMP levels and dampens the inflammatory signaling that drives skin plaques and joint inflammation. It doesn't require blood monitoring and has a straightforward titration schedule over the first week to minimize GI side effects.

PDE4 inhibition

Blocks phosphodiesterase 4 inside immune cells, elevating cyclic AMP and dampening the overproduction of pro-inflammatory cytokines—TNF-alpha, IL-17, IL-23—that drive the chronic skin and joint inflammation of psoriasis and psoriatic arthritis.

Oral non-biologic mechanism

Provides targeted anti-inflammatory activity at the intracellular level through an oral pill rather than injectable biologic therapy—offering meaningful disease control without the immunosuppression depth and monitoring requirements of systemic biologics.

No monitoring or refrigeration required

Unlike many psoriasis biologics that require regular blood tests and cold storage, Otezla requires no lab monitoring and is stored at room temperature—simplifying the treatment experience for patients managing chronic disease long-term.

Frequently Asked Questions

How is Otezla different from biologic medications for psoriasis?

Otezla is an oral pill — not an injection — and doesn't require blood monitoring or refrigeration. It works by blocking PDE4 inside immune cells rather than targeting specific inflammatory proteins like biologic medications do.

How long before Otezla improves psoriasis or psoriatic arthritis?

For psoriasis, visible skin improvement typically develops over 4–8 weeks. For psoriatic arthritis, joint symptom improvement may be noticed sooner. Full benefit continues building over 4–6 months of consistent use.

Is there a dose titration when starting Otezla?

Yes. Otezla is started at a lower dose and increased over the first week to minimize GI side effects. The starter pack guides this titration — following it carefully significantly improves early tolerability.

What side effects might I experience?

Diarrhea, nausea, headache, and upper respiratory infections are the most commonly reported effects — particularly during the initial titration period. These GI effects often improve after the first 2–4 weeks of use.

Does Otezla affect mood?

Depression and suicidal thoughts have been reported in patients taking Otezla — particularly those with prior psychiatric history. Report any new or worsening depression, anxiety, or mood changes to your clinician promptly.
