What is Opzelura (Ruxolitinib)?

Opzelura is a prescription topical cream containing ruxolitinib, a JAK inhibitor used to treat atopic dermatitis (eczema) and vitiligo. It works by blocking JAK1 and JAK2 enzymes that drive the inflammatory and immune signaling responsible for these skin conditions. For eczema, it reduces the itch, redness, and inflammation of flares. For vitiligo, it helps repigment areas of skin that have lost their color due to immune destruction of melanocytes. It's applied twice daily to affected areas and represents a targeted, mechanism-specific approach to inflammatory skin disease.

JAK1/JAK2 inhibition

Blocks Janus kinase enzymes that transmit inflammatory cytokine signals within skin cells, interrupting the immune-driven inflammation and immune cell activation that cause both eczema flares and vitiligo depigmentation.

Itch and inflammation relief

By dampening the JAK-STAT signaling pathway in the skin, Opzelura rapidly reduces the intense itch, redness, and skin barrier disruption characteristic of atopic dermatitis flares.

Vitiligo repigmentation

For vitiligo, JAK inhibition helps reverse the immune attack on melanocytes and allows pigment-producing cells to recover and repopulate depigmented patches—a mechanism distinct from any previously available vitiligo treatment.

Frequently Asked Questions

How quickly does Opzelura work for eczema?

Many people notice significant itch relief within the first 1–2 weeks of twice-daily use. Visible skin improvement in redness and lesions typically develops over 4–8 weeks of consistent application.

How is Opzelura different from topical steroids?

Opzelura works through a non-steroidal mechanism — blocking JAK enzymes that drive inflammation. This means it can be used on sensitive areas without the skin-thinning risks associated with long-term topical corticosteroid use.

Can Opzelura be used for both eczema and vitiligo?

Yes. Opzelura is FDA-approved for both atopic dermatitis and vitiligo — treating eczema inflammation and helping repigment areas affected by vitiligo through the same JAK-inhibiting mechanism.

What side effects might I experience?

Local application site reactions — burning, stinging, and redness — are most common. Because some ruxolitinib is absorbed systemically, there is a small increase in infection risk with larger treatment areas.

Is there a limit to how much skin area I can treat?

Yes. Treatment should be limited to no more than 20% of body surface area to limit systemic absorption. Your clinician will advise on the appropriate area to treat based on your condition.
