What is nummular eczema and how is it different from regular eczema?

Nummular eczema (nummular dermatitis) presents as distinct, coin-shaped patches of inflamed, itchy, sometimes oozing skin — most often on the legs, arms, and trunk. Unlike atopic dermatitis, it doesn't necessarily follow an atopic pattern and can occur in people without a history of eczema or allergies.

What causes nummular eczema?

The exact cause isn't fully understood. Triggers include very dry skin, skin injury or irritation, certain medications, contact allergens, and sometimes bacterial colonization (particularly Staphylococcus aureus). It's more common in older adults and during winter months when skin tends to be drier.

Is nummular eczema contagious?

No — nummular eczema is not infectious or contagious, even when patches appear weepy or crusted. Secondary bacterial infection of the patches can occur, which requires antibiotic treatment, but the underlying condition itself is not transmissible.

How long does a nummular eczema flare last?

Without treatment, flares can persist for months. With appropriate topical steroid treatment and aggressive moisturization, most patches improve within weeks — though recurrence is common. Identifying and addressing personal triggers reduces frequency and duration of future flares.

What side effects are associated with nummular eczema treatments?

Topical corticosteroids can cause skin thinning and pigmentation changes with prolonged use — particularly on sensitive areas. Non-steroidal alternatives including tacrolimus and crisaborole are preferred for longer-term use. Antibiotics for secondary infection are generally well tolerated at appropriate courses.
