What is Nucala?

Nucala is a biologic injection used as an add-on maintenance treatment for severe eosinophilic asthma, eosinophilic granulomatosis with polyangiitis (EGPA), and hypereosinophilic syndrome. It works by targeting interleukin-5 (IL-5)—the cytokine primarily responsible for the growth, activation, and survival of eosinophils, the white blood cells that drive eosinophilic inflammation. By blocking IL-5, Nucala dramatically reduces circulating and tissue eosinophil counts, decreasing the underlying inflammation that causes severe asthma attacks and organ damage in eosinophilic conditions. It's given as a subcutaneous injection once every four weeks.

IL-5 cytokine blockade

Binds to and neutralizes interleukin-5, the primary growth and survival factor for eosinophils—preventing IL-5 from binding to its receptor on eosinophil progenitors and mature cells, dramatically reducing eosinophil production and survival.

Eosinophil count reduction

Clinical trials showed reductions in blood eosinophil counts of greater than 80% with Nucala—directly reducing the tissue eosinophilia responsible for airway inflammation in severe asthma and organ involvement in EGPA.

Asthma exacerbation prevention

By controlling the eosinophilic inflammation driving severe asthma, Nucala significantly reduces the frequency of severe asthma attacks and the need for oral corticosteroid rescue courses—improving quality of life and reducing long-term steroid burden.

How long before Nucala reduces my asthma attacks?

Clinical trials showed meaningful reductions in exacerbation rates within the first 6 months. Many patients also experience progressive reduction in oral steroid requirements — an important quality-of-life benefit tracked over time.

How is Nucala given?

Nucala is injected subcutaneously once every 4 weeks — at a clinic, healthcare provider's office, or at home after proper training. The 100 mg dose is standard for asthma; EGPA uses a different dosing protocol.

What side effects might I experience?

Headache, back pain, fatigue, injection site reactions, and upper respiratory infections are the most commonly reported effects. Herpes zoster (shingles) reactivation has been reported — vaccination before starting is worth discussing.

Will Nucala replace my other asthma medications?

No — Nucala is an add-on therapy used alongside existing controller and rescue medications, not a replacement for them. Over time, your clinician may be able to reduce oral corticosteroid doses if eosinophil levels and symptoms improve.
