What is Novolin N (Insulin isophane)?

Novolin N is an intermediate-acting NPH insulin used to provide background blood sugar control for people with Type 1 or Type 2 diabetes. Unlike rapid-acting insulins that cover mealtime glucose spikes, Novolin N works more slowly—onset around 1–2 hours, peak effect at 4–12 hours, and duration up to 18–24 hours—providing coverage between meals and overnight. It's a human insulin suspension that must be gently rolled before each injection to evenly distribute the insulin crystals. It can be used alone or mixed with short-acting regular insulin to provide both background and mealtime coverage.

Intermediate-acting basal coverage

NPH insulin's protamine suspension creates a depot under the skin that releases insulin slowly over hours, providing gradual background glucose control between meals and overnight rather than the rapid action of mealtime insulins.

Human insulin formulation

Novolin N is identical in structure to human insulin, produced by recombinant DNA technology—a well-established, widely available formulation used for decades across all diabetes types.

Mixing compatibility

Can be mixed with short-acting regular insulin in the same syringe, allowing patients to administer both background and mealtime insulin coverage in a single injection—reducing the number of daily injections required.

Frequently Asked Questions

What type of insulin is Novolin N and when does it peak?

Novolin N is intermediate-acting NPH insulin. It typically starts working 1–2 hours after injection, peaks at 4–12 hours, and lasts up to 18–24 hours — providing extended background coverage between meals and overnight.

Why do I need to roll the Novolin N pen or vial before use?

NPH insulin is a suspension — the active insulin particles settle over time. Rolling or gently tipping (not shaking) the pen or vial before each use ensures uniform distribution for a consistent dose.

Can I mix Novolin N with regular insulin?

Yes. NPH insulin can be mixed with regular human insulin in the same syringe — draw regular insulin first. Do not mix NPH with insulin analogs like NovoLog, Humalog, or Lantus in the same syringe.

What side effects might I experience?

Hypoglycemia (low blood sugar) is the primary risk — particularly during the peak activity window of 4–12 hours after injection. Symptoms include shakiness, sweating, confusion, and rapid heartbeat. Injection site reactions can also occur.

How is Novolin N different from long-acting insulins like Lantus?

Novolin N has a more pronounced peak effect, shorter duration, and more variable action than modern long-acting analogs like Lantus or Levemir. It requires more careful timing relative to meals and activities.
