What is Nocdurna (Desmopressin)?

Nocdurna is a low-dose sublingual desmopressin tablet specifically formulated to treat nocturia—waking up one or more times during the night to urinate—due to nocturnal polyuria (producing too much urine at night). It works by mimicking ADH (antidiuretic hormone), the natural hormone that signals the kidneys to concentrate urine and reduce urine output during sleep. Nocdurna comes in sex-specific doses—a lower dose for women and a higher dose for men—reflecting differences in how the drug is metabolized. It's placed under the tongue at least one hour before bed.

ADH receptor activation

Binds to V2 receptors in the kidney collecting ducts, mimicking the natural antidiuretic hormone signal that causes the kidneys to reabsorb water and concentrate urine—reducing nighttime urine production.

Nocturnal polyuria targeting

Specifically addresses the nighttime overproduction of urine that disrupts sleep, rather than treating overall bladder capacity or urgency—making it distinct from other overactive bladder medications.

Sex-differentiated dosing

Available in separate formulations for men and women based on pharmacokinetic differences—women are prescribed a lower dose due to slower clearance, which reduces the risk of hyponatremia (low sodium) that can occur with excessive desmopressin activity.

Frequently Asked Questions

What is Nocdurna specifically approved for?

Nocdurna is approved for nocturia due to nocturnal polyuria — producing too much urine at night — in adults. It is not approved for daytime urinary frequency or urgency, which require different treatments.

Why are the doses different for men and women?

Women metabolize desmopressin more slowly than men, meaning they achieve higher drug levels at the same dose. Sex-specific dosing — a lower dose for women — reduces the risk of hyponatremia (dangerously low sodium levels).

What is hyponatremia and why does it matter with Nocdurna?

Hyponatremia is low blood sodium that can cause headache, nausea, confusion, and in severe cases seizures. Desmopressin reduces urinary water loss, which can dilute sodium. Limiting fluid intake before bed and at night is essential.

What side effects might I experience?

Headache, nausea, and hyponatremia are the most important side effects to monitor. Avoid drinking fluids for at least one hour before taking Nocdurna and limit intake overnight to reduce hyponatremia risk.

Who should not use Nocdurna?

People with heart failure, uncontrolled hypertension, kidney disease, or conditions causing fluid or sodium imbalance should not use Nocdurna. Age over 65 also requires extra caution due to higher hyponatremia risk.
