What is Nitrostat (nitroglycerin sublingual)?

Nitrostat is a fast-acting sublingual nitroglycerin tablet used to provide rapid relief of acute angina attacks—the chest pain that occurs when the heart muscle doesn't get enough oxygen-rich blood. It's placed under the tongue where it dissolves and absorbs quickly into the bloodstream, beginning to work within 1–3 minutes. Nitroglycerin works by relaxing and widening both the coronary arteries and the veins returning blood to the heart, reducing cardiac workload and improving blood flow to the heart muscle. It can also be taken just before activities known to trigger angina.

Nitric oxide-mediated vasodilation

Nitroglycerin is converted to nitric oxide in vascular smooth muscle, activating guanylate cyclase and causing both venous and arterial relaxation—reducing preload, afterload, and myocardial oxygen demand while improving coronary blood flow.

Sublingual rapid onset

Dissolving under the tongue delivers nitroglycerin directly into the sublingual veins for rapid systemic absorption, bypassing first-pass liver metabolism and providing onset of action within 1–3 minutes—essential for acute angina relief.

Rescue and prophylactic use

Used both to treat angina attacks as they occur and to prevent them when taken 5–10 minutes before activities known to trigger symptoms—providing flexible, on-demand cardiovascular protection.

Frequently Asked Questions

How do I use Nitrostat correctly during a chest pain episode?

Place one tablet under the tongue and let it dissolve — do not swallow it. Sit or lie down. If pain doesn't improve after 5 minutes, take a second tablet. After a third tablet at 5-minute intervals, call 911 immediately.

Why must I sit or lie down when taking Nitrostat?

Nitroglycerin causes rapid vasodilation that can significantly lower blood pressure — standing suddenly after taking it can cause dizziness or fainting. Sitting or lying down prevents dangerous falls.

How should I store Nitrostat?

Store in the original amber glass container away from heat, light, and moisture. Do not transfer to plastic containers — nitroglycerin degrades quickly when exposed to air, heat, or light. Replace every 6 months or at expiration.

What side effects might I experience?

Throbbing headache, flushing, dizziness, and a burning or tingling sensation under the tongue are common and expected — the headache confirms the medication is working. These effects are usually brief and resolve within minutes.

Can I take Nitrostat if I use erectile dysfunction medications?

No. Combining nitroglycerin with PDE5 inhibitors (Viagra, Cialis, Levitra, Stendra) can cause a severe and potentially life-threatening drop in blood pressure. This combination is absolutely contraindicated — always inform any treating clinician.
