What is the difference between nightmares and night terrors?

Nightmares occur during REM sleep — causing vivid, frightening dreams that typically awaken the sleeper with clear recall of the dream. Night terrors (sleep terrors) are NREM parasomnias — occurring in deep sleep, with the person appearing agitated or screaming but being difficult to wake and having no dream recall. They are more common in children.

When do nightmares require clinical evaluation?

Evaluation is appropriate when nightmares are frequent (multiple times per week), causing significant distress or sleep avoidance, associated with daytime PTSD symptoms, beginning after a new medication, or severely disrupting sleep quality. Nightmare disorder — clinically significant nightmare frequency — has effective treatments.

Can medications cause nightmares?

Yes — beta-blockers, certain antidepressants, melatonin at high doses, varenicline, and some blood pressure medications are known to cause vivid dreams and nightmares. If nightmares began around a medication change, a clinical review is appropriate. Often, timing adjustment or a switch to an alternative resolves the problem.

What treatments are available for PTSD-related nightmares?

Prazosin — an alpha-1 blocker — has the strongest evidence for reducing PTSD-related nightmares and improving sleep. Image rehearsal therapy (IRT) — a cognitive behavioral technique involving rewriting and mentally rehearsing a revised dream — is highly effective and durable. Trauma-focused therapy addresses the underlying PTSD.

What side effects are associated with nightmare treatments?

Prazosin may cause low blood pressure and dizziness — starting at a low dose minimizes this. Image rehearsal therapy carries no physical side effects. REM sleep behavior disorder (a different parasomnia with dream-enactment behaviors) is treated with clonazepam or melatonin, each with their respective side effect considerations.
