Evaluation is appropriate for frequent, severe, or worsening cramps, cramps associated with weakness, numbness, or leg color changes with exertion (suggesting vascular disease), cramps in the context of known diabetes, kidney, or thyroid disease, or when a medication cause is suspected.

What medications can cause night cramps?

Diuretics (potassium loss), statins, beta-agonists, calcium channel blockers, and certain antidepressants are among the medications most commonly associated with nocturnal cramps. A medication review is an important early step in evaluation — adjusting or substituting a contributing medication may resolve the problem.

What stretches or lifestyle changes help with night cramps?

Regular calf stretching before bedtime (standing calf stretch held for 30 seconds, repeated several times) has good evidence for reducing cramp frequency. Staying well hydrated, reducing caffeine and alcohol, and maintaining adequate magnesium intake through diet or supplementation are practical supportive measures.

What side effects are associated with night cramp treatments?

Magnesium supplements may cause diarrhea — magnesium glycinate is better tolerated. Quinine — once commonly used — is no longer recommended due to serious cardiac and hematologic risks. Muscle relaxants may cause sedation. Stretching and hydration carry no side effects and are appropriate first-line interventions.
