Frequently Asked Questions

What types of musculoskeletal problems can be managed through primary care?

Most acute soft tissue injuries (sprains, strains, tendinopathy), common joint pain (osteoarthritis, bursitis), mechanical back and neck pain, overuse injuries, and minor sports injuries are well-managed in primary care. Complex fractures, ligament tears requiring surgery, and conditions not responding to conservative management benefit from specialist referral.

When should musculoskeletal pain be evaluated by a specialist?

Referral is appropriate for suspected fracture, complete ligament or tendon rupture, locked or acutely unstable joint, neurological involvement (weakness, numbness), inflammatory joint disease (swollen, warm, multiple joints), pain not improving after 6–8 weeks of appropriate conservative management, or when imaging findings require subspecialty interpretation.

What is the role of physical therapy in musculoskeletal care?

PT is the cornerstone of most musculoskeletal rehabilitation — addressing muscle imbalances, restoring range of motion, building functional strength, improving movement mechanics, and reducing re-injury risk. Early PT referral consistently produces better long-term outcomes than delayed referral or passive treatment alone.

Do I need imaging for most musculoskeletal problems?

Most acute soft tissue musculoskeletal problems are diagnosed clinically without imaging. X-rays are used when fracture is suspected. MRI and ultrasound are reserved for soft tissue pathology not resolving with conservative management or when surgical planning is required. Routine imaging rarely changes initial management.

What side effects are associated with common musculoskeletal treatments?

NSAIDs carry GI and renal risks — take with food and limit duration. Corticosteroid injections provide medium-term relief but may cause tendon weakening with repeated injections. Physical therapy is very safe — expected muscle soreness after sessions resolves quickly. Surgical treatments carry standard procedural risks discussed at the time of referral.
