What is Monurol (Fosfomycin)?

Monurol is an oral antibiotic prescribed to treat uncomplicated urinary tract infections in women, taken as a single 3-gram powder packet dissolved in water. Its standout feature is its convenience—one dose is the entire treatment course. Fosfomycin works by blocking a key step in bacterial cell wall synthesis, killing the bacteria responsible for the UTI. It's particularly useful for treating UTIs caused by bacteria that have become resistant to other antibiotics, including some strains of E. coli and Enterococcus faecalis. Symptom improvement is typically felt within 2–3 days.

Cell wall synthesis inhibition

Blocks MurA enzyme—an early and essential step in bacterial peptidoglycan cell wall construction—preventing bacteria from building and maintaining the structural wall they need to survive and replicate.

Single-dose treatment

The entire antibiotic course is one packet mixed in water and taken once, eliminating the multi-day compliance challenge of standard UTI antibiotic regimens—with no doses to forget or skip.

Resistance-active coverage

Fosfomycin's unique mechanism of action means it retains activity against many antibiotic-resistant UTI pathogens, including some ESBL-producing E. coli strains that have become resistant to commonly used oral antibiotics.

Frequently Asked Questions

Is one packet really the complete treatment?

Yes — a single 3-gram packet dissolved in water is the full course of treatment for most uncomplicated UTIs in women. There are no follow-up doses required, making it one of the simplest antibiotic regimens available.

How quickly will my UTI symptoms improve?

Most people begin noticing symptom improvement within 24–48 hours of the single dose. Full resolution typically occurs within 3–7 days. Contact your clinician if symptoms persist beyond a week.

Can Monurol treat UTIs caused by antibiotic-resistant bacteria?

Yes. Fosfomycin retains activity against many bacteria resistant to commonly used UTI antibiotics — including some ESBL-producing strains — making it a valuable option when standard antibiotics haven't worked.

What side effects might I experience?

Diarrhea, nausea, and headache are the most commonly reported effects — generally mild given the single-dose format and brief exposure. Vaginal yeast infection following antibiotic use can also occur in some women.

Can men use Monurol for UTIs?

Fosfomycin can be used in men for certain UTIs, but uncomplicated UTI is less common in men and may require longer courses or additional evaluation. Your clinician will determine the appropriate regimen.
