What is Memantine (Namenda)?

Memantine is a prescription medication used to treat moderate to severe Alzheimer's disease. It works through a different mechanism than cholinesterase inhibitors like donepezil—rather than boosting acetylcholine, it regulates glutamate activity in the brain, which becomes overactive in Alzheimer's and contributes to neuronal damage and cognitive decline. By moderating this excess glutamate signaling, Memantine helps preserve cognitive function, slow symptom progression, and maintain daily functional abilities for longer. It's taken once or twice daily and is often used alongside a cholinesterase inhibitor for a complementary dual approach.

NMDA receptor modulation

Blocks NMDA glutamate receptors in a voltage-dependent, use-dependent manner—reducing the pathological glutamate overactivation that contributes to excitotoxic neuronal damage in Alzheimer's disease while allowing normal glutamate signaling to continue.

Cognitive function preservation

By reducing excitotoxic damage, Memantine helps slow the decline in memory, thinking, and daily function associated with moderate-to-severe Alzheimer's—not reversing the disease, but slowing its progression.

Complementary to cholinesterase inhibitors

Works through a completely different mechanism than donepezil, rivastigmine, and galantamine—making it an effective add-on therapy that targets Alzheimer's pathophysiology through two independent pharmacological pathways simultaneously.

Frequently Asked Questions

What stage of Alzheimer's is Namenda used for?

Namenda is approved for moderate to severe Alzheimer's disease — the later stages where symptoms have progressed beyond what cholinesterase inhibitors alone can adequately address. It fills a gap where earlier-stage medications fall short.

Can Namenda be used alongside Aricept or other cholinesterase inhibitors?

Yes. Namenda and cholinesterase inhibitors work through different mechanisms and are frequently prescribed together for a complementary approach to managing moderate to severe Alzheimer's symptoms.

How will I know if Namenda is working?

The goal is preserving function and slowing decline rather than improvement. Caregivers and clinicians typically assess whether daily function and cognition are declining more slowly than expected without treatment.

What side effects might I experience?

Dizziness, headache, confusion, and constipation are the most commonly reported effects. Confusion — though it sounds counterintuitive — can occur early in treatment, particularly if the dose is increased too quickly.

Does Namenda cure Alzheimer's disease?

No. Namenda does not cure or reverse Alzheimer's — no medication currently can. It helps slow the rate of cognitive and functional decline in some patients, helping maintain independence and quality of life for longer.
