What is Megace (Megestrol)?

Megace is a synthetic progestin used primarily to stimulate appetite and promote weight gain in patients experiencing significant weight loss and anorexia associated with cancer, HIV/AIDS, or other serious illnesses. At the doses used for appetite stimulation, it works through hormonal and metabolic pathways that increase hunger and improve caloric intake. It's also used in some cases to treat certain hormone-sensitive breast and endometrial cancers. Available in tablet and oral suspension form, it's taken once or multiple times daily depending on the indication.

Appetite stimulation

At higher doses, Megestrol activates appetite-regulating pathways in the hypothalamus and may suppress pro-inflammatory cytokines like TNF-alpha and IL-6 that contribute to the anorexia and metabolic wasting seen in serious illness.

Weight and lean mass support

Promotes caloric intake and helps reverse unintentional weight loss in patients with cancer- or HIV-related cachexia—improving nutritional status, energy levels, and overall quality of life.

Hormonal anti-tumor activity

As a progestin, Megestrol can inhibit the growth of hormone receptor-positive breast and endometrial cancers by opposing estrogen's stimulating effects on tumor cell proliferation.

Frequently Asked Questions

How quickly does Megace improve appetite?

Many patients notice increased appetite within the first 2–4 weeks of starting Megace. Weight stabilization or gain may take longer, typically 4–8 weeks of consistent daily use.

Is Megace a steroid?

Megace is a synthetic progestin — related to the hormone progesterone — not a traditional anabolic or corticosteroid. However, at higher doses used for appetite stimulation, it can have some glucocorticoid-like effects.

Can Megace be used for cancer treatment as well as appetite?

Yes. At standard doses it's used for appetite stimulation and weight management in serious illness. It's also used at different doses to treat hormone receptor-positive breast and endometrial cancers.

What side effects might I experience?

Fluid retention, blood clot risk (DVT and PE), adrenal suppression with long-term use, and blood sugar elevation are the most significant concerns. These risks increase with dose and duration of use.

Does Megace interact with other medications?

Yes. Megace can interact with blood thinners, insulin, and other medications. Your clinician will review your full medication list and monitor for relevant interactions during treatment.
