Frequently Asked Questions

What do different types of lower back pain indicate?

Localized muscle aching after activity suggests strain. Sharp pain radiating down the leg (sciatica) suggests nerve root compression from disc herniation or stenosis. Pain worse with walking and relieved by leaning forward suggests spinal stenosis. Pain at rest or at night, fever, or unexplained weight loss are red flags requiring prompt evaluation.

When should lower back symptoms prompt urgent evaluation?

Seek same-day or emergency evaluation for lower back pain with new bowel or bladder dysfunction (especially urinary retention or incontinence), saddle area numbness, bilateral leg weakness, or pain following significant trauma. These may indicate cauda equina syndrome — a neurological emergency requiring immediate intervention.

Do I need an MRI for my lower back?

Most acute lower back symptoms do not require immediate imaging — findings often don't change initial management and may cause unnecessary anxiety about incidental age-related changes. MRI is indicated for red flag features, progressive neurological deficits, or failure to improve after 4–6 weeks of appropriate conservative management.

What is the difference between a herniated disc and degenerative disc disease?

A herniated disc occurs when the disc nucleus pushes through the outer ring — compressing nearby nerves and causing radicular symptoms. Degenerative disc disease is a gradual aging process involving disc height loss and desiccation — often asymptomatic but can contribute to chronic lower back pain and nerve canal narrowing (stenosis).

What side effects are associated with lower back symptom treatments?

NSAIDs carry GI and renal risks. Muscle relaxants cause significant drowsiness. Gabapentinoids for radicular pain may cause sedation and dizziness. Epidural steroid injections can cause temporary pain flare and blood sugar elevation. Opioids are not recommended for non-specific back pain. Physical therapy is safe and produces better long-term outcomes than passive treatments. Lower Back Symptoms visit What to expect from a Lower Back Symptoms visit
