How do I know if my back pain is a muscle strain or something more serious?

Muscle strain typically follows a specific activity or mechanical trigger, improves with rest and movement modification, and has no neurological features. Red flags requiring prompt evaluation include pain radiating down the leg, numbness or weakness, bladder or bowel changes, significant trauma, unexplained weight loss, or pain that's worse at rest or at night.

Do I need imaging for lower back pain?

Most acute lower back pain does not require imaging — findings on X-ray or MRI often don't change management and frequently show age-related changes unrelated to current symptoms. Imaging is indicated when red flag features are present or when symptoms aren't improving as expected after 4–6 weeks of appropriate management.

What is the best way to recover from a lower back strain?

Staying active — avoiding bed rest — is consistently supported by evidence. Gentle movement, continuing normal activities as tolerated, heat for muscle spasm, NSAIDs or acetaminophen for pain, and physical therapy for persistent cases are the evidence-based approach. Prolonged rest delays recovery.

When should I see a physical therapist for back pain?

Physical therapy is beneficial for subacute or recurrent lower back pain — particularly when symptoms persist beyond 2–4 weeks or when functional limitations are significant. Early PT has better outcomes than delayed referral, and reduces the risk of transitioning to chronic pain.

What side effects are associated with lower back strain treatments?
