What is Lokelma (Sodium zirconium cyclosilicate)?

Lokelma is a prescription medication used to treat hyperkalemia—abnormally high potassium levels in the blood—a potentially dangerous condition that can cause life-threatening cardiac arrhythmias. It works by trapping potassium ions in the gastrointestinal tract and removing them from the body through the stool, before they can be absorbed. Unlike older potassium binders, Lokelma has a highly specific and efficient binding structure that selectively captures potassium throughout the entire GI tract. It acts quickly—lowering potassium levels within hours—and can be used for both acute correction and ongoing maintenance to keep potassium in a safe range.

Selective potassium ion trapping

Lokelma's microporous zirconium silicate structure acts as an artificial ion exchanger, selectively capturing potassium ions (as well as ammonium and hydrogen ions) throughout the GI tract—removing them via the stool before absorption into the bloodstream.

Rapid onset potassium reduction

Begins lowering serum potassium within 1–2 hours of the first dose, making it one of the fastest-acting oral potassium-lowering agents—useful for both urgent correction of acute hyperkalemia and as maintenance therapy.

Maintenance tolerability

Unlike older potassium binders (like sodium polystyrene sulfonate) that have significant GI tolerability issues, Lokelma is generally well-tolerated for long-term daily use—important for patients with chronic kidney disease or heart failure who require ongoing potassium management.

Frequently Asked Questions

What is Lokelma used to treat?

Lokelma treats hyperkalemia — dangerously elevated blood potassium levels that can cause life-threatening heart rhythm abnormalities. It works by trapping potassium ions in the gut and removing them through the stool before absorption.

How quickly does Lokelma lower potassium?

Lokelma begins lowering serum potassium within 1–2 hours of the first dose — one of the fastest-acting oral potassium-lowering options available. This rapid onset makes it useful for both acute correction and ongoing maintenance.

Can Lokelma be used long-term to prevent hyperkalemia?

Yes. Lokelma is approved for both acute management of high potassium and as ongoing maintenance therapy in patients with chronic conditions — like kidney disease or heart failure — that predispose them to recurring hyperkalemia.

What side effects might I experience?

Edema (swelling) due to the sodium content of Lokelma is the most clinically significant concern — particularly for patients with heart failure or restricted sodium intake. Hypokalemia (low potassium) from over-treatment is also possible.

How do I take Lokelma?

Mix the powder packet in water and drink immediately. For the acute correction phase, it's taken three times daily for up to 48 hours, then reduced to once daily for maintenance — always on an empty stomach, 2 hours before or after other medications.
