What is Lifitegrast (Xiidra)?

Xiidra is a prescription eye drop used to treat the signs and symptoms of dry eye disease, specifically targeting the underlying inflammatory process rather than simply lubricating the eye surface. It works by blocking LFA-1, a protein on the surface of immune cells that promotes inflammation in the tear glands and eye surface. By interrupting this inflammatory signaling, Xiidra reduces the immune-driven damage that disrupts tear production and eye surface health. It's applied twice daily and may take up to 12 weeks to produce the full benefit.

LFA-1 integrin blockade

Blocks the interaction between LFA-1 on T-lymphocytes and ICAM-1 on eye surface cells, preventing the immune cell activation and inflammatory cascade that damages the ocular surface and reduces tear production in dry eye disease.

Root-cause anti-inflammatory treatment

Rather than masking symptoms with artificial lubrication, Xiidra addresses the underlying inflammatory mechanism driving dry eye—improving both the symptoms patients feel and the clinical signs providers measure.

Dual symptom and sign improvement

Clinical trials demonstrated improvement in both patient-reported eye dryness and discomfort and objective measures of ocular surface damage—distinguishing it from lubricating drops that only relieve symptoms temporarily.

Frequently Asked Questions

How long before Xiidra improves my dry eye symptoms?

Some patients notice symptom improvement within 2 weeks, but meaningful relief of both the symptoms and underlying inflammation typically takes 6–12 weeks of consistent twice-daily use.

How is Xiidra different from Restasis?

Both treat dry eye disease by addressing inflammation, but through different mechanisms. Xiidra blocks LFA-1, while Restasis uses cyclosporine. Some patients respond better to one than the other — clinicians may try both.

Do I need to use artificial tears alongside Xiidra?

Artificial tears can still be used for immediate symptom relief while Xiidra builds its anti-inflammatory effect. Wait at least 15 minutes between applying Xiidra and other eye drops.

What side effects might I experience?

Eye irritation, burning or stinging upon instillation, blurred vision shortly after application, and an unusual taste in the mouth after using the drops are the most commonly reported effects.

Can I wear contact lenses while using Xiidra?

Remove contact lenses before applying Xiidra and wait at least 15 minutes before reinserting them. The preservative in the drop can be absorbed by soft contact lenses and cause irritation.
