What causes laryngitis?

Acute laryngitis is most commonly caused by viral upper respiratory infection — causing inflammation and swelling of the vocal cords. Other causes include voice overuse, acid reflux affecting the larynx (laryngopharyngeal reflux), bacterial infection (less common), and inhalation of irritants. Chronic laryngitis lasting more than 3 weeks requires evaluation for persistent causes.

Do I need antibiotics for laryngitis?

The vast majority of laryngitis cases are viral — antibiotics provide no benefit and are not routinely indicated. Bacterial laryngitis is uncommon in adults. If laryngitis is suspected to be bacterial based on clinical features (high fever, purulent exudate, severe sore throat), clinical evaluation determines whether antibiotics are appropriate.

How should I rest my voice during laryngitis?

Voice rest — speaking as little as possible and avoiding whispering (which strains the cords more than quiet speech) — is the primary treatment. Stay well hydrated and use a humidifier. Avoid throat clearing, which traumatizes already-inflamed cords. Caffeine and alcohol increase mucosal dryness and are best avoided.

When should hoarseness from laryngitis prompt evaluation?

Any hoarseness persisting beyond 2–3 weeks — particularly in a smoker, someone with significant alcohol use, or a professional voice user — warrants laryngoscopic evaluation to visualize the vocal cords and rule out nodules, polyps, or more serious pathology.

What side effects are associated with laryngitis treatments?

Voice rest and hydration carry no side effects. PPIs for reflux-related laryngitis are well tolerated at appropriate durations. Corticosteroids — sometimes used for severe laryngitis affecting the airway — carry short-course side effect profiles including insomnia and blood sugar effects. Throat lozenges are very safe.
