What is a keloid and how is it different from a hypertrophic scar?

A keloid is an overgrowth of scar tissue extending beyond the original wound boundaries — often continuing to grow over time. A hypertrophic scar stays within the wound margins and may regress spontaneously. Keloids are more common in individuals with darker skin tones and have a strong genetic predisposition.

What triggers keloid formation?

Keloids can develop after any skin injury — surgery, piercings, tattoos, acne, burns, or even minor scratches. High-risk body sites include the earlobes, shoulders, chest, and upper back. People with a personal or family history of keloids should be counseled about this risk before elective procedures.

Can keloids be permanently removed?

Keloid treatment is challenging — no single approach has a high cure rate, and recurrence after any treatment is common, often with a larger keloid than the original. Combination approaches — intralesional corticosteroid injection plus silicone sheeting, or post-surgical radiation — produce the best outcomes for most patients.

What treatments are available for keloids?

Intralesional corticosteroid injections (triamcinolone) are first-line — typically given every 4–6 weeks, flattening the keloid over multiple sessions. Silicone gel sheets, cryotherapy, laser therapy, and surgical excision combined with post-operative adjuvant treatment are other options. Combination approaches outperform single modalities.

What side effects are associated with keloid treatments?

Intralesional steroid injections may cause skin atrophy, pigmentation changes, and telangiectasias at the injection site. Cryotherapy causes temporary pigmentation changes — particularly in darker skin. Surgical excision alone has high recurrence rates.
