What is Januvia (sitagliptin)?

Januvia is a DPP-4 inhibitor used to improve blood sugar control in adults with Type 2 diabetes. It works by blocking the enzyme that breaks down incretin hormones—GLP-1 and GIP—allowing these natural hormones to remain active longer and stimulate insulin release in a glucose-dependent manner. This means Januvia only promotes insulin secretion when blood sugar is elevated, significantly reducing the risk of hypoglycemia compared to older diabetes medications. It's taken once daily as a tablet and is weight-neutral, making it a well-tolerated addition to existing diabetes regimens.

DPP-4 enzyme inhibition

Blocks dipeptidyl peptidase-4, the enzyme that rapidly degrades GLP-1 and GIP incretin hormones, extending their activity and allowing them to stimulate more insulin release and suppress glucagon in a glucose-dependent manner.

Glucose-dependent insulin stimulation

Because it works through incretin hormones that only activate insulin release when blood sugar is elevated, Januvia carries a low risk of hypoglycemia—unlike sulfonylureas that stimulate insulin regardless of current glucose levels.

Weight-neutral profile

Unlike some diabetes medications that cause weight gain, Sitagliptin has a neutral effect on body weight—an important consideration for Type 2 diabetes management where excess weight is often a contributing factor.

Frequently Asked Questions

How does Januvia lower blood sugar differently from metformin?

Januvia works by inhibiting DPP-4 to extend the activity of natural incretin hormones — stimulating insulin release only when blood sugar is elevated. Metformin reduces liver glucose production and improves insulin sensitivity through entirely different pathways.

Does Januvia cause weight gain?

Januvia is weight-neutral — it neither causes weight gain nor promotes weight loss. This distinguishes it from sulfonylureas (which cause weight gain) and GLP-1 agonists (which often promote weight loss).

Can Januvia be used alongside other diabetes medications?

Yes. Januvia is frequently added to metformin, sulfonylureas, insulin, or other diabetes medications for additional blood sugar control. Dose adjustments of other medications may be needed when combining.

What side effects might I experience?

Upper respiratory infections, stuffy or runny nose, headache, and sore throat are the most commonly reported. Joint pain has been reported in some patients. Serious pancreatitis has been reported rarely — seek care for severe persistent abdominal pain.

Does Januvia require dose adjustment for kidney disease?

Yes. Unlike Tradjenta (linagliptin), sitagliptin is primarily cleared by the kidneys and requires dose reduction as kidney function declines. Your clinician will adjust the dose based on your eGFR.
