What is Intuniv (Guanfacine)?

Intuniv is an extended-release, non-stimulant prescription medication approved for ADHD in children and adolescents aged 6–17. It works by selectively activating alpha-2A adrenergic receptors in the prefrontal cortex—the brain region responsible for attention, impulse control, and working memory. By strengthening these neural connections, Intuniv improves focus and reduces hyperactivity and impulsivity without the stimulant mechanism of medications like Adderall or Ritalin. It's taken once daily and may be used alone or alongside stimulant ADHD medications. It also has a calming effect that some patients find beneficial for emotional regulation.

Alpha-2A receptor activation in prefrontal cortex

Strengthens the signaling of postsynaptic alpha-2A adrenergic receptors in the prefrontal cortex, enhancing the neural network activity responsible for sustained attention, working memory, and behavioral inhibition.

Non-stimulant mechanism

Works through adrenergic receptor modulation rather than dopamine release or reuptake inhibition, making it a non-controlled substance with no abuse potential and a distinctly different side effect profile from amphetamine-based medications.

Emotional regulation and calming

Beyond core ADHD symptoms, Guanfacine's prefrontal effects can reduce emotional dysregulation, irritability, and hyperarousal—benefits that are particularly relevant for children with ADHD and co-occurring emotional or behavioral challenges.

Frequently Asked Questions

How is Intuniv different from stimulant ADHD medications?

Intuniv is a non-stimulant — it works through alpha-2A adrenergic receptors in the prefrontal cortex rather than dopamine pathways. It's not a controlled substance, doesn't cause the same appetite suppression, and builds effect gradually over weeks.

How long before Intuniv improves ADHD symptoms?

Meaningful improvement in attention, impulse control, and hyperactivity typically takes 4–6 weeks as the dose is gradually increased. Full benefit may take up to 8 weeks to establish at the optimal dose.

Can Intuniv be used alongside stimulant ADHD medications?

Yes. Intuniv is sometimes used as an adjunct to stimulant therapy — particularly when emotional dysregulation, hyperactivity, or tics are prominent features. Your clinician will determine the right combination approach.

What side effects might I experience?

Drowsiness, low blood pressure, dizziness, fatigue, and stomach upset are most common, particularly when starting or increasing the dose. Drowsiness often improves after the first few weeks as the body adjusts.

Can I stop Intuniv suddenly?

No. Stopping guanfacine abruptly can cause rebound hypertension and other withdrawal effects. Always taper the dose gradually under your clinician's supervision when discontinuing treatment.
